` *Position Summary:* * The Care and Service Case Management Coordinator is responsible for providing integrated support, advocacy, and coordination of care from engagement to post-discharge for clients and families in a integrated setting.
The position will facilitate activities to recruit, engage, and retain clients and families in program activities and services.
*Job Duties:* * Screen, assess, and evaluate clients’ economic and psychosocial needs.
* Provide outreach to clients to facilitate their engagement in services and offer additional support so they can engage in treatment and live successfully in the community.
* Connect clients to community services, including medical, behavioral, residential, entitlement, and/or any other needed services per interdisciplinary care plan.
* Coordinate care planning with other providers of services/resources to ensure goal-directed, collaborative care (including care transitions).
* Provide education on the client’s physical and/or mental health condition(s) and ways to live a healthy life.
* Provide coaching and recovery support, especially on issues of self-advocacy, linkage to aftercare, wellness self-management, medication adherence, smoking cessation, entitlements, and daily living activities.
* Accompany clients to medical and/or other appointments to facilitate their engagement and follow through as needed.
* Assist clients with connecting to services via in-person or telehealth.
* Maintain documents, records, statistics, and other related reports in an organized, timely, and accurate manner in accordance with policy and procedure.
* Function as a resource to team members on community partners/services.
* Complete training and maintain proficiency with de-escalation and crisis intervention techniques.
* Establish and maintain positive working relationships with others, both internally and externally.
* Work cooperatively and effectively with others to foster teamwork, set goals, resolve problems, and make decisions.

Bachelor’s Degree in Social Work or another related field preferred.
* At least two years of knowledge, familiarity, and/or experience in case management services.
* Bilingual in English and another language preferred.
Desired languages mirror our client populations and may include Spanish, Arabic, and/or Kurdish.
* Demonstrated skills and competencies to work with individuals regarding a variety of behavioral health needs.
* Knowledge of the functions and services of community organizations and related human services.
*Physical Requirements:* * Hybrid work environment (in office and remote).
* Will provide services in a field setting.
* Valid driver’s license, liability auto insurance, and ability to drive personal vehicle for work.
* Travel to various locations to support client services, as needed and as applicable by region.
* Laptop and company-issued cell phone.
* Must be able to work a flexible schedule, including evenings and/or weekends (as needed).
* Occasional lifting of office items no more than fifty (50) pounds.
